Here are the steps to retrieve your username:
- From the portal login page, click on "Forgot username / password"
- In the Forgot Username section, enter your email address and click"Submit"
- A message containing your username will be emailed to you
Notes:
- The email will be sent immediately
- If you do not receive the email in your inbox, check your spam folder
- The email address used must match the email address used to set up your account. If the email is not received, it is likely you have a different email. Please contact your study coordinator who can update your email address and provide your username.
If you cannot contact your study coordinator, contact StudyTRAX Support by either sending an email to support@studytrax.com or submitting a ticket at https://studytrax.zendesk.com/hc/en-us/requests/new
Please provide as much of the following Information as you have available when contacting StudyTRAX Support:
- Project name
- The link used to access StudyTRAX
- Study Coordinator name
- Principal Investigator name
- Research Institute
